The referral agreement is a signed acknowledgement made by and between EXPERT SECOND OPINION SERVICES PVT. LTD., and Hello ESO Associate, in which they agree to coordinate patient care between the primary care physician and the specialist physician.

EXPERT SECOND OPINION SERVICES PVT. LTD. has a documented referral process and procedure. At the time of the referral, Second party (Associate) will register with First party(Hello ESO) and First party(Hello ESO) will generate the code and send the code to the second party(Associate).

Second party(Associate) provide the following details:

• Consult Request (Reason for the Associate)
• Patient detail including history of presenting complaint, examination findings and investigation results
• Current medications, problems and allergies of the patient.
• Most recent progress note.
• Any applicable labs, imaging or testing results.

Significant details to the patient’s care will be exchanged through written communication.

If the patient does not show up for their initial appointment, First party will notify the referring physician.

First Party and Second party both warrant to keep confidential the information of patients and information obtained from the discussion, signing and performing this Agreement (including business secrets, co-operation plans, operational activities, financial information, technical intelligence, operational informational and other business secrets). Second party must not disclose all or part of the documents and information to any third party without the permission of First party unless otherwise regulated by the law or regulation or agreed by the both the parties.
